Yorkshire General Histopathology EQA Scheme
The scheme is open to Histopathologists at Consultant, Staff Grade and Associate Specialist Grade who report general surgical histopathology independently.
Specialist Trainees are also encouraged to participate in the EQA for educational purposes.
The scheme consists of two circulations per annum, one in the Spring, usually February-March, and one in the Autumn, usually September-October. Each circulation comprises ten H&E stained histology sections, which have been submitted by the participants and deemed suitable for EQA by the Organising Committee.
The scheme is CPA accredited and receives periodic inspection visits to retain accreditation.

Exemption Form
Current circulation
The next circulation will be Circulation X and is due to commence in March 2012.
Please can all participants ensure that their exemptions are up to date prior to the circulation commencing. Participants may update their exemptions by completing the Exemption Form (listed under General Items) and submitting it to the EQA Facilitator.

Initial Results
Once the EQA circulation is closed the submitted diagnoses are collated to produce the initial range of diagnoses for each case in the EQA. The initial results of the EQA are discussed at the participants meeting prior to the production of the final EQA results.

The Participants' Meeting
After each circulation a meeting is held that is open to all participants. At this meeting the cases in the circulation are discussed and the participants at the meeting decide whether the case should be included in the scoring system and which, if any, of the submitted diagnoses can be merged. The final results for the EQA are produced based on the decisions made at the participants meeting.
The Participants' Meeting for Circulation X will be held on the afternoon of Monday 21st May 2012 in the Level 7 Lecture Theatre, Bexley Wing, St James's University Hospital

Application for HEQA - Spring 2012
If you are interested in joining the Yorkshire General Histopathology EQA Scheme, please take the time to read through the Participants Manual and complete and sign the relevant enrolment form below. The enrolment form should be posted to the EQA Facilitator at Quality Assurance Reference Centre, Ground Floor, Raynham House, 2 Capitol Close, Capitol Park West, Leeds, LS27 0WH.
On receipt of your enrolment form the EQA Facilitator will allocate you a confidential participant code to allow participation in the EQA.
